Pre-existing conditions

Declare every known condition when you register. Chronic conditions common in older adults (such as hypertension, diabetes and arthritis) are managed under the plan's terms, subject to underwriting. Non-disclosure can lead to cancellation or rejected claims.

First-year conditions

The schedule lists a first-year waiting period for some surgeries, cancer care and intensive care services. Exact conditions, including renewal and continuity, are confirmed at enrolment.

Who can enrol in Clear Elite?

Adults aged 50 to 75, subject to underwriting. You can register a parent, another relative, or yourself if you're in that age range.

Can I enrol my parent from abroad?

Yes. Many sponsors live in the UK, US, Canada and elsewhere and register a parent in Nigeria. You fill in the form online, Azuka confirms the details with you on WhatsApp, and the member ID is shared with the family.

How much does it cost?

₦600,000 per person per year, for a global limit of up to ₦3,000,000 a year. That's about ₦1,650 a day. Each person needs their own registration.

What does Clear Elite cover?

Consultations with 24 specialist types, laboratory tests, X-rays and ultrasound, CT and MRI twice a year, echocardiography once a year, private-ward admission up to 30 days, ICU for 48 hours, surgery up to ₦500,000, chemotherapy up to ₦450,000, acute dialysis up to ₦200,000, eye care, dental care, physiotherapy, psychiatric sessions, telemedicine, a second opinion from specialists abroad and an annual health check. The official 2026 benefit schedule is the controlling document.

Are there waiting periods?

Yes. Admissions and minor surgery open in quarter 1, primary dental care and physiotherapy in quarter 2, eye care in quarter 3 and intermediate surgeries in quarter 4. Chronic-disease medication starts after 2 months, optical and dental after 3 months, and funeral support after 6 months. Some surgeries, cancer care and ICU have first-year conditions.

Burns over 9%, congenital abnormalities, home care, hormone replacement therapy, joint replacements and prosthetics, neurological and neurosurgical disorders, cosmetic surgery, self-inflicted injuries, speech disorders, thyroid disorders, transplants, obesity treatment, virility drugs, and anything not expressly listed in the schedule.

Are pre-existing conditions covered?

Declare every known condition when you register. Chronic conditions common in older adults, such as hypertension, diabetes and arthritis, are managed under the plan's terms, subject to underwriting. Not declaring a known condition can lead to cancellation or rejected claims.

Which hospitals can my parent use?

A Clearline provider close to where they live, chosen during registration and confirmed at activation. Use the hospital finder on this site to search more than 2,000 provider records by state, city and type.

How do I pay, and how do I know it's safe?

No payment is needed to register. After Azuka confirms eligibility, pay only into the official company account: Polaris Bank, Clearline International Limited, 1771104431. Never pay into a personal account. Then send the receipt.

How soon does cover start?

After payment is confirmed, the member ID and policy documents usually follow within 5 working days. The waiting periods above then apply.

What happens at the hospital?

Show the member ID at the HMO desk. The hospital requests an authorisation code from Clearline for anything beyond a basic consultation. If there's a delay, ask the hospital to send the request and call the Clearline contact centre on 0700 245 245 245.

What if the hospital asks for a deposit?

For covered care, call the contact centre straight away with the member ID and the hospital's name, and ask them to speak with the hospital. In a genuine emergency, life comes first: get the patient stabilised, then sort out the paperwork.

Can I register both parents?

Yes. Complete one registration for each person. The premium is per person.

Is my information safe?

Your details are used only for enrolment, underwriting and claims, and are never sold. Optional measurement is off unless you allow it, and medical, ID and payment details are never used for measurement. See the privacy notice.

  • Guide download: optional name and WhatsApp/email if you choose to share them. You can download without them.
  • Registration: the enrollee's personal, contact, hospital and health details, optional ID and photo, next of kin, and optional payment details. Used for enrolment, underwriting and claims.

Where it goes

Form submissions are received through Netlify Forms and reviewed by the ClearlineHMO enrolment team. We don't sell personal data. Health information is only used for enrolment, underwriting and care coordination.

Cookies & optional measurement

The site works with necessary features only. If you choose Allow optional measurement, a Meta Pixel measures which campaigns bring visitors and whether key steps are completed (for example "registration completed").
